About The Role:

The Support Coordinator/Program Facilitator will provide support coordination to NDIS participants, helping them to navigate their way through the NDIS and supporting them to achieve their personal goals. You will demonstrate person-centred practice by ensuring clients are able to make informed choices about their service provision. 

You will also facilitate outcome-oriented educational, fun, and relevant skill development programs for individuals and small groups of people with disability and complete any reporting and documentation as required. The role will be offered on a part time basis, working four days per week based at our Ridleyton office. 

About You:

As a valued member of the team, you will have demonstrated experience working with clients with disability and be highly person-centred. You will hold a Certificate IV in Disability and have a high degree of creativity and flair. You will be an excellent communicator and must be able to work with a range of internal and external stakeholders. You will be highly organised and flexible with great time management skills and must be willing to undertake some regional travel as required. 

Core Responsibilities:

  • Facilitate a range of programs for individuals and small groups to increase independence and skill development
  • Deliver programs that are suited to the individual and in line with their personal goals
  • Support people to learn functional skills that can be adapted to daily living
  • Develop a variety of Easy English and pictorial resources
  • Engage with clients and their decision maker to ensure they understand their NDIS plan 
  • Connect clients to providers and assist them with their plan
  • Work with clients to address any barriers, crisis/conflict as required
  • Document evidence of service delivery and progress notes with a high degree of accuracy
  • Willing to undertake regional travel as required

Skills and Experience:

  • Demonstrated experience working with clients with disability
  • Highly committed to applying a person-centred practice
  • Have strong networking skills and able to work with internal and external stakeholders 
  • Strong communication skills in both written and verbal form
  • Effective time management and organisational skills

Requirements:

  • Certificate IV in Disability
  • Current DHS – Working with Children Check or willing to obtain
  • NDIS Worker Check or willing to obtain
  • National Police Clearance or willing to obtain
  • Current First Aid and CPR Certificate
